list_decisions

Paginate / filter: sincedate, filepattern, protectedonly, tags, includesuperseded.

record_decision

Capture a decision. donotrevert=true triggers Claude Code PreToolUse enforcement; symbol="login" scopes the lock to one function/class. It supersedes a strong unprotected near-duplicate instead of appending a twin.
